内皮脂肪酶与冠心病的相关性分析

   【摘要】  目的 检测冠心病患者血浆内皮脂肪酶(EL)水平,探讨EL与冠心病的相关性。 方法 选取81例冠心病患者,并根据临床表现分为稳定性心绞痛(SAP)(n=18)、不稳定性心绞痛(UAP)(n=23)、急性心肌梗死(AMI)(n=21)及对照组(n=19)。所有患者分别进行病史采集,并检测空腹血糖(FBG)、血脂、高敏C反应蛋白(hs?CRP)、EL水平,分析EL与年龄、性别、吸烟比例、HDL?C、LDL?C、FBG、hs?CRP及冠状动脉病变程度的相关性。 结果 对照组血浆EL水平为(346.4±54.3)μg/L,而SAP组、UAP组、AMI组分别为:(693.2±89.7)μg/L、(780.3±99.2)μg/L、(962.5±123.3)μg/L,各组间EL水平比较差异有统计学意义(P<0.05);EL与HDL?C、LDL?C、FBG及hs?CRP明显相关(P<0.05);Gensini评分SAP组分别与UAP组、AMI组比较,差异有统计学意义(P<0.01),冠心病组EL平均水平与冠状动脉狭窄程度Gensini平均评分呈正相关(r=0.483,P<0.01)。 结论 冠心病患者体内EL表达明显增高,EL在冠心病的发生中可能起一定的作用。

    Abstract:Objective  To investigate the correlation between plasma endothelial lipase (EL) level and coronary heart disease (CHD).    Methods  A total of 81 patients were enrolled and divided into four groups: stable angina pectoris group (SAP, n=18), unstable angina pectoris group (UAP, n=23), acute myocardial infarction group (AMI, n=21) and control group (n=19). Clinical history was collected and fasting blood glucose(FBG), high?density lipoprotein?cholesterol(HDL?C), low?density lipoprotein?cholesterol(LDL?C), high sensitive?C reactive protein(hs?CRP) levels were determined. Relevance of EL to age, sex, smoking proportion, HDL?C, LDL?C, FBG, hs?CRP levels and coronary lesion was analyzed.   Results  Plasma EL level was (346.4±54.3) μg/L in the control group, (693.2±89.7) μg/L in SAP group, (780.3±99.2) μg/L in UAP group and (962.5±123.3) μg/L in AMI group. The differences among the four groups were statistically significant (P<0.05). Significant correlation between EL level and HDL?C, LDL?C, FBG and hs?CRP levels were found (all P<0.05). Gensini scoring of SAP group was significantly different from the other three groups (all P<0.05). There was positive correlation between EL level and coronary lesion (r=0.483 P<0.01).   Conclusions  EL expression is increased in CHD patients. EL might play an important role in the genesis of CHD.

    在冠心病患者尸体解剖中,发现内皮脂肪酶(endothelial lipase,EL)在冠状动脉粥样硬化斑块的内皮细胞、平滑肌细胞、巨噬细胞中均存在表达[1],但是冠心病患者体内EL的表达水平及EL水平与冠心病病变程度的研究,目前国内外报道甚少。本研究对冠心病患者的血浆EL水平进行了检测,并与冠心病的危险因素及冠状动脉病变程度相关性进行了分析,报道如下。
1  对象和方法
    1.1  研究对象
    选取2006年9月至2007年9月山西医科大学第二医院心内科冠状动脉造影确诊的冠心病患者,分别进行病史采集,检测空腹血糖、血脂、血清高敏C反应蛋白(hs?CRP)及进行心电图检查。排除(1)急慢性感染、创伤或手术2周以内;(2)合并脑血管意外或外周血管疾病、肝肾功能不全、恶性肿瘤、慢性结缔组织疾病或免疫性疾病。
    1.1.1  诊断标准  冠心病确诊条件为:(1)根据典型的临床症状、心电图改变以及心肌酶谱变化而确诊的AMI或OMI。(2)经心电图证实有明确心肌缺血的典型心绞痛发作史。(3)冠状动脉造影证实存在冠状动脉狭窄≥50%病变并有心肌缺血心电图改变或典型心绞痛发作史。
    1.1.2  分组  根据冠心病患者临床表现、生化检查、心电图和冠状动脉造影结果将入选的81例患者分为:稳定性心绞痛组(SAP组,n=18)、不稳定性心绞痛组(UAP组,n=23)、急性心肌梗死组(AMI组,n=21)及对照组19例,对照组临床表现或心电图变化怀疑冠心病,经选择性冠状动脉造影术证实无冠状动脉狭窄。
